Special instructions:
This permission form may contain words that are new to you. If you read any words that are not clear to you, please ask the person who gave you this form to explain them to you or contact the project director.
Inclusion Criteria:
Youth ages 12-18
Must live in Missoula County
Purpose:
You are being asked to give permission for your child to take part in a research study assessing barriers to engaging in pro-social programs/activities (like afterschool clubs). This study aims to identify perceptions of program accessibility, relevance, relationships, safety, and structural barriers.
Procedures:
If you agree, your child will join a group of other youth ages 12-18 to discuss their experiences (or lack thereof) with pro-social programs and activities in Missoula County. These sessions will be audio recorded, and will last approximately 90 minutes. These sessions will take place at the Missoula Public Health (301 West Alder St) or Missoula Public Library.
Payment for Participation:
Your child will be compensated for their time with a $50 gift card.
Risks/Discomforts:
Answering the questions may cause your child to think about feelings that make him/her sad or upset. You will be informed of any new findings that may affect your decision to allow your child to remain in the study. There is no anticipated discomfort for those contributing to this study, so risk to your child is minimal.
Benefits:
Your child’s help with this study will guide improvements to youth program design, outreach strategies, and policy advocacy to strengthen youth engagement across Missoula County.
Confidentiality:
All records will be kept confidential and will not be released without your consent except as required by law. Only the researchers and their faculty supervisor will have access to the files, recordings, and transcripts associated with this research. Both your and your child’s identity will be kept private. If the results of this study are written in a scientific journal or presented at a scientific meeting, neither your nor your child’s name will be used.
Voluntary Participation/Withdrawal:
Your decision to allow your child to take part in this research study is entirely voluntary. Your child may leave the study for any reason.
Your child may be asked to leave the study for any of the following reasons:
1. Failure to follow the Project Director’s instructions;
2. The Project Director thinks it is in the best interest of your child’s health and welfare; or
3. The study is terminated.
Future research:
Identifiers might be removed from the identifiable private information and could then be used for future research studies or distributed to another investigator for future research studies without additional parental permission from you or the child’s legal guardian.
